Why a Probiotic Needs a Prebiotic (Most Don't Have One)

Here is a formulation question worth asking before you buy any probiotic: once these bacteria arrive, what are they going to eat?

Most labels have no answer. A probiotic with no prebiotic is a delivery without a destination.

The two halves

A probiotic is the live organism. A prebiotic is the fibre that feeds it, and which your own digestive enzymes cannot break down — that is what makes it available to bacteria in the first place. Put both in one product and it is called a synbiotic.

Bacteria that arrive in a gut with nothing to ferment do not establish well. They pass through. You paid for CFUs that left.

Why CFU is the most over-weighted number on the label

CFU means colony-forming units — how many live organisms are in a dose. Brands compete on it because it is a big number and easy to print.

It tells you almost nothing on its own. What matters more:

  • Which strain, named specifically. Different strains of the same species do different things. "Contains Lactobacillus" is roughly as informative as "contains a dog".
  • Survival to the colon. Stomach acid and bile kill most organisms on the way. A product that does not address this is shipping most of its CFUs to be destroyed in transit.
  • CFU at end of shelf life, not at manufacture. These are very different numbers and only one of them is the one you will swallow.
  • Whether there is anything to eat on arrival. The point of this article.

The Akkermansia case, specifically

This one is unusual and worth understanding, because it is counterintuitive.

Akkermansia muciniphila does not eat fibre. It eats mucin — the mucus lining of your own gut. That sounds destructive and isn't: consuming mucin stimulates the goblet cells in your gut wall to produce more of it, which is why a healthy Akkermansia population is associated with a thicker, better-maintained barrier rather than a thinner one.

So why pair it with inulin at all?

Because inulin does not feed Akkermansia directly. It feeds the wider bacterial community that produces short-chain fatty acids, keeps the gut environment hospitable, and sustains the conditions in which the mucus layer is maintained. You are not feeding the animal. You are maintaining the habitat.

There is a second reason, and it is the one that changes buying decisions: Akkermansia is an anaerobe. Oxygen damages it. That is why handling and stabilisation matter more than the CFU count, and why much of the human trial data used a pasteurised preparation — which performed well, awkwardly for anyone selling live-only as inherently superior.

The prebiotics worth knowing

  • Inulin — from chicory root; the most studied, ferments fairly quickly.
  • FOS (fructooligosaccharides) — closely related, in onions, leeks, garlic.
  • GOS (galactooligosaccharides) — different bacterial targets, often better tolerated.
  • Resistant starch — from cooled cooked potato or rice; ferments further along the colon.
  • Partially hydrolysed guar gum — slow fermenting, often used when others cause too much gas.

They are not interchangeable, because they ferment at different rates and in different places.

Start low, or you will regret it

Fermentation produces gas. That is the mechanism working, but going from a low-fibre diet straight to a large inulin dose produces a genuinely unpleasant week and is the single most common reason people give up.

Start at a fraction of the label dose and build over two to three weeks. If a particular fibre keeps disagreeing with you, try a slower-fermenting one rather than abandoning the idea.

What to look for on a label

  • A named strain, not "proprietary blend". If the organism is not named, assume it is not meaningfully present.
  • A prebiotic fibre with a number next to it.
  • Something about how it is stabilised and what the CFU figure refers to.

Common questions

Can I just take inulin separately?

Yes, and for many people that is the sensible starting move — it is cheap and it feeds what you already have. Start low.

Do I need to take them at the same time?

Not strictly, but a combined product removes the question and the fibre is there when the organisms arrive.

Does yoghurt count?

As a probiotic source, partly. Not as a source of Akkermansia — it is not in yoghurt, and no amount of it will put it there.

Should I take probiotics with antibiotics?

Not at the same time of day, and ask your doctor about the course as a whole. Antibiotics do not distinguish between bacteria you want and bacteria you don't.
